Transform Health & Community was created to bridge a gap.
Too often, smaller health, community, and social service organizations are underserved by large consulting firms: priced out, deprioritized, or handed templated frameworks that don’t reflect their reality. At the same time, the challenges they face are complex, system-level, and high-stakes.
This practice was built to meet that need.

Meet Nadia Feerasta
M.S.W, MHSc., CHL
Nadia is a senior health system consultant who brings two decades of experience across clinical practice, hospital operations, and system‑level planning. She has worked with Ministries, Regional Health Authorities and individual organization to support system planning, funding model reform, organizational reviews and restructuring. Nadia helps organizations understand how their services function today and what needs to change to meet future demand.
Nadia works with leaders to define success, assess risks, and build plans that can be delivered within real constraints. She draws on Ministry priorities, clinical data, service utilization patterns, and operational insight to guide decisions on capacity, resource allocation, and performance. Her work has shaped region‑wide service models, redesigned care pathways, and supported providers through operational and financial stabilization.
Nadia’s leadership is grounded in clarity, evidence, and purposeful change. She brings a steady, facilitative approach to complex engagements and expertly guides groups through difficult conversations about roles, resources, and accountability. She creates space for clinicians, leaders, community partners, and patients/clients to surface concerns and priorities, then translates those inputs into clear decisions and workable steps.
Her thoughtful approach enables a shared understanding of operational realities and the development of practical solutions so that organizations can implement and sustain change, even in challenging contexts. Nadia’s track record demonstrates a clear commitment to supporting clients in moving from strategy to action.

Transform Health & Community operates as a focused, senior-led practice, intentionally designed to protect quality and depth. When projects require specialized expertise or additional capacity, we collaborate with a trusted network of experienced associates.
These are not junior staff or interchangeable contractors. They are seasoned professionals with deep expertise in areas such as evaluation, policy analysis, facilitation, and system design. Associates are selected intentionally based on the specific needs of each engagement.

Debra Carson
Debra Carson brings over 25 years of healthcare leadership experience in hospital and community sectors, including a decade at the Senior Leadership level.
While a senior leader at a major hospital, Debra was instrumental in leading multiple patient care programs in the development and implementation of new models of care, strategic planning, and quality improvement. She also led the clinical engagement process during the merger of two legacy hospitals.
She has been described as an authentic leader, a trusted mentor, and a compassionate individual while achieving/surpassing sustainable outcomes. Successes in numerous transformations are attributed to her strong engagement approach and facilitation skills with interprofessional teams.
She is a graduate of the University of Toronto for both her Masters, Health Administration and Bachelor of Science, Occupational Therapy.

Kavita Mehta
Kavita is a senior healthcare executive and public affairs leader with over 25 years of experience in primary care transformation, health policy, and organizational change.
She brings expertise in government relations, strategic planning, system design, and stakeholder engagement, with a strong track record of influencing policy and building cross-sector partnerships.
Kavita is known for translating complex policy and system challenges into practical, actionable solutions. Combining clinical insight, business acumen, and systems thinking, she helps organizations strengthen partnerships, secure funding, and implement sustainable, high-impact, patient-centered initiatives at scale.

Danielle Mccarron
Danielle McCarron is a high-impact healthcare executive and consultant with over a decade of experience driving operational excellence and
strategic growth across the mental health and addictions sectors in Canada, the U.S., and the UK.
Specializing in organizational turnarounds and system-level scaling, Danielle excels at translating complex clinical needs into sustainable, results-driven business models.

Katya Masnyk
Katya is an experienced regulator, executive leader, facilitator and researcher. She has committed her 20-year career to advocating for the public interest and championing patient voice.
Katya combines her executive leadership experience with her research, policy and planning expertise to champion collaborative leadership informed by big-picture analytical expertise and a steadfast commitment to quality improvement and innovation.

Not afraid to think outside the box, Katya has developed numerous strategic and operational plans focused on innovation, stakeholder engagement and proactive quality improvement, leading to recognition and federal government and regulatory awards.

Bill Croson
Bill Croson is a statistician with a career focus on health and social services analysis. His work is based on systems-level considerations, the continuum of care from public health and primary care to chronic disease management and palliative care in relation to a range of clinical specialty areas, the use of clinical and financial data, and the preparation of sophisticated quantitative analyses.
He has made extensive use of mixed methods approaches to gain a more comprehensive understanding of issues relevant to the projects he undertakes.
Bill’s experience includes working on complex clinical services and program planning projects across Canada from local level to provincial/territorial level initiatives.

Judith Rudoler
Judith Rudoler, MHA, CCAC, is a healthcare executive with over 15 years of progressive leadership across public and private healthcare systems.
She most recently served as Executive Director of one of Canada’s leading residential addiction treatment centres, with full accountability for organizational strategy, operational performance, accreditation compliance, and interdisciplinary leadership across inpatient and virtual care environments.
In prior executive roles, she led performance improvement and system redesign initiatives to strengthen frameworks, expand access, and ensure regulatory alignment. She has also led multi-year, multi-sector provincial health promotion initiatives within Public Health, delivering measurable community impact.
Judith holds a Master of Health Administration (Community Care) and is a Canadian Certified Addiction Counsellor, bringing disciplined systems leadership and clinical expertise to complex healthcare environments.
